This book was originally published by Brown & Benchmark publishers of Madison, Wisconsin in 1994. The book is now out of print and the rights reverted back to me. I have converted the first edition to HTML and PDF. I am making the first edition available free to qualifying educators for personal use. Do not distribute this text without my permission.
